Initial Enquiry

Choosing the right care setting can be a  difficult decision to make and we are here to support and advise you to help make the process a little easier. 

We will discuss your current needs and medical history, medical conditions, mental capacity and mobility in order to assess if we can cater to your needs. 

We will invite you to Palm Court to meet the Manager who will show you our facilities and a potential room. It is also a great opportunity to get a feel for our homely environment and meet some of the staff and residents. 

A breakdown of the proposed weekly fees will be sent in an offer letter to you.

Preparations to Welcome You

To ensure the smoothest of transitions from one care setting to ours, once terms and conditions have been agreed, we can immediately start to prepare for your arrival. 

You will be informed of the room you will be designated - in most cases, the one you viewed and accepted when you visited the home. 

We promote independence and individuality, so encourage everyone who joins our Well-Being Community to personalise their room as much as possible - photo frames, pictures we can hang on the wall, and any other trinkets that will bring comfort and familiarity to the room. 

We will liaise with you every step of the way,  and let you know when you can start bringing personal belongings to the home, if possible, ahead of your agreed admission date.   

We will start your Care Plan with the information we have so far gathered and notify the Nursing and Care teams that you will be joining us and will be aware of your care needs ahead of your arrival.

Admission Day

At the Morning Handover Meeting, new residents and their needs will be discussed and Room Number confirmed, so that all staff are prepared to give you a warm welcome, and one Senior Carer will be assigned to help you settle in.

An inventory of all your belongings will be recorded and filed in your Care Plan. 

Any valuables will need to be kept in a locked safe in the Manager's Office. It is also possible to store any cash or extra items such a cigarettes with the Manager. 

A Person-Centred Activities Questionnaire  will be given to you to complete so that we can create personalised activities that reflect your passions and interests. 

We can help you arrange transport to Palm Court on the day of admission and can book this in advance. 

Pre-Admission Assessments 

We work closely with Local Authority Brokerage teams that introduce us to potential residents who are funded by the Local Authority and a pre-admission assessment is organised. 

Medical history, medical conditions, mental capacity and mobility will be discussed in the assessment with the Discharge team of either a hospital or care home to identify individual care needs and preferences. 

Following the assessment, a weekly fee is proposed by the Home to cover all care requirements and agreed by the Local Authority or POA of the prospective client. 

We deal directly with self-funding clients and arrange a mutually agreed contract following a care assessment.  

Clinical Preparations 

We will liaise with the Discharging team of either the hospital or care home to ensure we have all the correct medication - we usually arrange for 1 week's supply to help with your transition to Palm Court. 
 
We will get in touch with your GP Surgery to inform them that you will be joining us or if you are coming from outside of Eastbourne, we will liaise with Enys Road Surgery and register you as a new patient. 

We will also get in touch with a Specialist Nurse such as Diabetes, Catheter or TVN  - Tissue Viability Nurse to arrange any necessary referrals or reviews. 

If necessary, we will also notify an external Nutritionist we work with who can create a Nutritional Support Plan that will be implemented with a Food Diary.  

We will ensure that your room will have the necessary equipment that you require such as a air/pressure mattress or sensor pads. 

If you require a motorised chair, you will need to bring or purchase one ahead of your admission. We will be able to help you arrange this if you need assistance. 

Assessments on Admission

When you arrive, we will carry out assessments in the first 48 hours to ensure the information we have collected is correct or if there has been a change in your care needs since the pre-admission assessment. 

We will seek your consent to display a photo of your face on your Care Plan and on the door to your room. We will require your consent to share your personal information with the Care team and other professionals who may be enlisted to help you. 

We will check and assess the following: 
- Your medication and prescribed dosages
- The health of your skin
- Your communication levels and needs
- Your mobility and the level of assistance you may need if you are at risk of falling. 
- Your nutrition and weight where your MUST score and BMI will be calculated.
- Your oral and dental health
- If you are diabetic, we will test your blood sugar levels to verify your normal range and symptoms of hypo/hyperglycaemia.

We aim to complete your Care Plan with further detailed risk assessments within the first month.
